Fatigue: Aviation’s Most Polite Safety Warning

Fatigue is one of aviation's most persistent safety challenges. While regulations place limits on duty and flight hours, tiredness can still affect pilots, cabin crew, and maintenance personnel in ways that degrade performance and increase risk. That is why modern aviation increasingly relies on fatigue risk management systems, reporting cultures, and organisational safeguards that go beyond simple compliance.

A roster can be perfectly legal, compliant with regulations, and within work limits, yet still leave personnel tired. Fatigue is not something that can be merely dismissed as tiredness. A fatigued professional is more prone to making mistakes, and those mistakes can be very costly when we are talking about airliners carrying hundreds of people every day.
IATA's Fatigue Risk Management material states that "crew member fatigue is now acknowledged as a hazard that predictably degrades various types of human performance and can contribute to aviation accidents and incidents."
To counter fatigue-related hazards, it is necessary for airlines to maintain strict limits on the flight time of pilots. Circadian rhythm, night duties, early starts, time-zone changes, sleep quality, commuting, disruption, and cumulative workload are all important factors that influence fatigue.
EASA has put an entire system in place to understand and mitigate the risks of fatigue. It is called the Fatigue Risk Management System (FRMS). EASA describes it as "a scientifically based, data-driven complement or alternative to the prescriptive regulation of flight and duty time and rest requirements." Legal compliance is only the starting point when trying to avoid fatigue-related hazards. FRMS helps organisations look deeper and gain a better understanding of the risks involved.
Fatigue management remains an active area of discussion at EASA, and its rules continue to evolve to cover a wide variety of fatigue-related issues. EASA states that improving fatigue risk management is a priority within the European Plan for Aviation Safety.
Opinion No. 02/2026, published by EASA in February 2026, outlines "harmonised and state-of-the-art flight time limitations and rest requirements" for commercial air transport operations using aeroplanes in emergency medical services, air taxi, and single-pilot operations.
When discussing safety limits on working hours, there is often a tendency to focus only on flying hours. However, we must also consider the fatigue experienced by maintenance engineers who may begin their shifts at three in the morning. They too are part of the safety chain, and a very critical part of it. Mistakes made during aircraft maintenance can have serious consequences once the aircraft enters service.
FAA Advisory Circular 120-115 "describes the basic concepts of human fatigue and how it relates to safety for aviation maintenance organisations and individual maintainers" and provides guidance on fatigue hazards and mitigation strategies specific to maintenance personnel. CASA's 2026 maintenance-sector material also treats fatigue as an important component of the maintenance safety-risk picture.
Fatigue rarely arrives with a knock on the door. It creeps into operations in the form of an incomplete cross-check, a missed detail during a handover, a shorter temper in the cabin, a less thorough troubleshooting process, a rushed logbook entry, or a decision made with less mental bandwidth than the operation deserves.
Reporting fatigue is also a difficult challenge because nobody wants to be the person who raises a hand and says, "I'm tired." The aviation industry must actively ensure that reporting fatigue is viewed as being just as important as reporting a technical issue. A poor airline culture that discourages personnel from reporting fatigue may, unfortunately, be creating a serious problem for itself.
Fatigue is more of a policy problem than many airline managers may wish to believe. They must recognise that fatigue risk cannot simply be outsourced to individual professionalism. Staffing levels, reserve coverage, turnaround assumptions, disruption buffers, simulator availability, maintenance manpower, and reporting culture all influence whether fatigue becomes a controlled risk or a hidden exposure. A fatigue programme that exists only on paper is not a fatigue programme at all. It is simply another layer of documentation added to compliance requirements.
Fatigue is a complex safety challenge that requires careful management at multiple levels. At its core lies one important question: Is the person actually fit to make the next safety-critical decision?
